What is Personalised Care?

Personalised care
is healthcare designed around the person — not just the condition. It means designing care with your patients, not for them.

Personalised care includes:

  • Building a partnership between clinicians and consumers, focused on what matters most to each individual

  • Using Patient Reported Outcome Measures (PROMs) and Patient Reported Experience Measures (PREMs) to measure progress and success

  • Creating a personalised care plan based on the patient's goals, values and life context

Personalised care is supported by frameworks like the NSQHS Standard 2: Partnering with Consumers, and it's a key priority in delivering safe, high-quality, patient-centred care.

Why deliver Personalised Care?

Healthcare is evolving — and so are patient expectations. Patients want to be equal partners in their care. They want personalised care services that respond to their priorities, not just clinical targets.

When you deliver personalised care, you:

  • Improve outcomes by aligning treatment with what matters most to the patient

  • Build trust, increase satisfaction and reduce misunderstandings

  • Use PROMs and PREMs to track progress and drive continuous improvement

  • Align with evolving healthcare standards like Standard 2: Partnering with Consumers

Delivering Personalised Care gives you the practical skills to meet these expectations confidently - with minimal complexity added to your already busy consultations.

What you'll learn.

Through four simple webinars and our range of 'how-to' resources, you’ll gain a practical understanding of:

  • The meaning of personalised care and why it matters

  • What it takes to partner with patients and consumers in meaningful ways

  • How to apply PROMs and PREMs in practice to measure the success of personalised care

  • How to embed this approach into your everyday patient consultations
